Went on a Invisalign 12 month treatment. From start to finish, Raj explains every step in incredible detail, answered all my concerns and makes you feel at ease. Regular catch ups over lockdown to ensure I was on track and timelines in place to measure progress. Forma go above and beyond to ensure you get what you want, quite accommodating to ensure you are comfortable… very hands on, and makes you feel safe with the treatment. Really pleased with the end results and happy that this was such a stress free experience. Amazing service! Would absolutely recommend.

I couldn’t recommend Dr. Raj more for Invisalign; he listens carefully to what your concerns are and what you are comfortable with. I went to Forma Smile for Invisalign treatment and I’m really happy with the outcome of my treatment. Dr Raj was always flexible when making appointments and was really helpful throughout my treatment. We worked on a treatment plan to go through some of my insecurities with my smile and even made some adjustments to the first plan. I had a few adjustments to my aligners after finishing my first round of aligners which was really great to have. At the end of my treatment I also had teeth whitening and was given three retainers to keep. I’d highly recommend Forma Smile if you’re looking to get Invisalign.

My wife was in a lot of pain due to growing wisdom teeth and Dr. Raj Kumar graciously accepted to see us on the same day. The surgical tooth extraction was done very cleanly and my wife recovered quite quickly due to it. It was a slightly complicated thing and he kept us updated on his process before and after the treatment. He is an experienced dentist who really goes the extra mile in treating his patients. Forma was the first clinic I went to. Dr Raj Kumar was extremely informative, professional and friendly. I started treatment there and then. As a child I had multiple uncomfortable, ugly braces and therefore never completed the treatment. The invisalign retainers are amazing. They are comfortable, practically invisible and removable. I would highly recommend Dr Raj and invisalign if you are thinking of having your teeth fixed. Growing up I had severely crowded and crooked teeth and had always wanted to get it fixed. Before deciding on invisalign and getting the treatment from Dr Kumar, I thought I would have to get braces if I wanted to get straight teeth. I went to four different places for consultation before meeting Dr Kumar and was told I would have to have 4 teeth removed and would have to have braces for 4 years plus the treatment cost was more expensive than I thought it would be. The consultants had a look at my teeth for about 5 minutes and gave their opinion. I was not happy with that and so kept researching and looking for other options. That's when I found Dr Kumar. I emailed him and was able to book an appointment for a consultation easily. When I met Dr Kumar, he was very approachable, friendly and made me feel at ease instantly. The consultation lasted over an hour and he didn't try to sell me or force me to get the treatment at all. He showed me the result of his previous patients and was very realistic and honest about how my treatment will follow. I was told that my treatment will take about 2 years and will only need to take 1 teeth out! Plus the treatment was more affordable than getting braces! I was happy to hear all this and decided to get the treatment from Dr Kumar. The treatment took about 3 years to complete, a little longer than originally planned but a few months ago I finished the invisalign treatment and now I have got straighter teeth and I am very happy with the result. Now when I look in the mirror and see my teeth, it feels surreal because I never thought I would have straight teeth. So if you are contemplating about getting a treatment to get your teeth finally fixed then I would say go, do it!

I suffered from severe class II malocclusion (12 mm gap!), Consulted several dentists in both my country and UK and was repeatedly told that it would have not been possible to correct this without undergoing jaw surgery in addition to extensive dental treatment. I was delighted when Dr Kumar told me I'd be able to get good results through two simple extractions and wearing Invisalign. It was a jump of faith and first few months were scary but I am now so happy I have done it because my smile and teeth looks x20 times better! Amazing results from Raj @ Forma Smile & Beauty. I had lost my confidence to smile; self-conscious of my teeth which had moved since fixed braces as a teenager. My friends told me they were fine!! But with Invisalign & Raj's careful attention to detail, I can now grin with pride! Just see my before & after photo. I bought the full package, and had multiple new scans and aligners due to a stubborn few teeth. This was all included in the initial price, which took any stress out of the process. Raj also identified a wisdom tooth abscess which had been missed at routine dental checks, I was able to get this sorted & stop intermittent pain I was experiencing.

Since I was a teenager I’d always hated my teeth (one in particular!) the overcrowding and general crookedness meant I had never smiled with my mouth open and felt I had to laugh with my hand over my mouth :-( After seeing an extremely unflattering off guard picture of myself from my 31st birthday party I decided to go for an Invisalign consultation. I chose Raj as years before he had done an amazing job on my best friends teeth and she couldn’t recommend him enough. November 14th 2017 was the day of my consultation, and from that very first day I felt informed and confident that I was making the right decision. I had check ups every 6-8 weeks and started to notice changes within the first couple of months. My boyfriend was so impressed by my treatment that he also decided to go to forma for his treatment :) It’s been just under 2 years since my first consultation and my journey has just come to an end, and I still don’t believe that girl in the mirror smiling back is me!! I cannot thank Raj and Henrietta enough for everything, the results far exceeded my expectations of what could be done with my old teeth! The procédure was very straight forward, nicely done and practically pain free. I went climbing the following day and got away with two pain killers and almost no bleeding. Dear Forma, I've now completed my Invisalign treatment - my buttons came off last week hoorah! I just wanted to say how simple and discreet the process was. I had aligners for 18months which included three Refinements and not once did anyone notice or mention them, they were so discreet. I also have a job which is customer facing and I was never conscious of them. On this basis alone I would recommend Invisalign but before I undertook the treatment I did a little research. Even though I could have gone to a dentist closer to home I chose to travel to London to Dr Kumar. He spent time explaining the whole treatment with case studies and expectations etc. over the course of 18 months not once did I have a problem arranging an appointment between aligners when I needed one. I decided last year that I wanted to get invisalign braces. My teeth were really crowded and my the top and bottom arches didn't even line up properly. I had been to see loads of dentists already, some of whom said my teeth couldn't be fixed with invisalign and that I needed to have traditional traintrack braces. After reading the positive reviews online, I decided to book a consultation with Dr Kumar at forma. He was really friendly and knew exactly what he was talking about. He showed me pics of similar cases he had done in the past and I was sold. I even managed to get the price down by £500 since I'm a student and thats what we do best :L I'm now on my 37th aligner and due to finish in 4 weeks. The braces have been fantastic and the outcome is amazing! I have seen Dr Kumar regularly for the last 18 months and he's always been fantastic. Even when things weren't going quite right (some of my teeth weren't moving as they should have been) Dr Kumar knew exactly what to do and was able to put me at ease. Last year, I was told I would lose one of my front teeth, owing to root resorption following a bike accident. Always petrified of visits to the dentist, I was dreading the day of extraction, implant and stitches. Furthermore, the procedure was expected to be complicated by likely breaking of the tooth, requiring the root to be removed separately, as well as potential damage to particularly thin bone in front of the tooth. However, Raj made me feel relaxed and managed to remove the tooth and root in one piece, without disturbing vulnerable bone. In fact, mid-surgery, when I asked how he was getting on, he replied that he had just completed the extraction. I hadn’t felt a thing! I was also amazed that, post-surgery, just one Diclofenac painkiller was all I needed to take, as I felt absolutely no discomfort afterwards.
